Person-centered personal assistance for adults 21 and older with physical disabilities — delivered with caring, respect, and love. Long Term Care lets you remain in your own home, and lets you choose the person who cares for you.

  • Personal hygiene — bathing, washing hair, grooming, shaving, nail care, foot care, skin care, and mouth care

  • Assistance with movement, mobility, and repositioning
  • Assistance with medications
  • Meal preparation
  • Light household tasks related to the person's needs
  • Grocery shopping

Consumers can choose their own caregiver to care for them. Elite handles the paperwork, pays caregivers weekly, and offers overtime at competitive rates.

Covered through OLTL waivers and Community HealthChoices. Program eligibility confirmed before care starts.
